Patient Registration Form

Depending on the type of examination you will be having, you may save time by downloading and filling out this patient registration form and faxing it to 626-821-3460 or bringing it with you to your appointment.

Our office hours are Monday - Friday, 8:00 a.m. to 5:00 p.m. We are open during lunch for your convenience. Patients are welcomed on a "walk-in" basis for certain exams, although appointments are encouraged.

Depending on the type of exam ordered by your physician, our schedulers will provide specific preparation instructions needed to complete the exam.

Patients should bring their insurance card, the exam order form filled out by their physician and any prior studies that relate to that day's exam.

When you arrive at our office, one of our receptionists will verify that the information we have is complete and accurate. Your satisfaction is important to us and we will do everything possible to make sure your visit is a positive experience.
